Why is my employer providing these resources?

Your employer is committed to your personal and professional well-being. They want to support you in your effort to lead a happy and healthy life by providing tools and resources to help make that happen.

What is the reward for participating?

Rewards for participating vary by employer. Contact your employer representative for a detailed explanation of available rewards and when they will be distributed.

Who is eligible to participate?

Your employer determines who is eligible to participate. Some programs may require you and your spouse/domestic partner to participate to achieve rewards. Check with your employer if you are unsure of the requirements.

How do I get involved?

Participants must create an account or sign in to begin.

Where do I find the Wellness Assessment?

The Health Assessment can be found by clicking the Toolkit link after you log in to your dashboard. 

What if I need help taking my Wellness Assessment?

Call 866-415-7138 for assistance with your Health Assessment. The Health Assessment is available in print upon request.

What if my documentation is out of date?

Results must be current and within required time frames. Out-of-date documentation will not be accepted.

What if my results are submitted past the deadline?

Late results will not be accepted. We recommend keeping track of your progress, looking ahead at program requirements and confirming results are submitted before the deadline.
